These are track listing inserts for standard retail DVDs that didn't come with them originally - or at least didn't come with them when I bought them.

Just save them to your computer, load them up in your printing program of choice, cut them out and stick them in the DVD case. They should print out about 5" x 7". Anything different and you need to either use a different program or play with the settings in the one you have.
